My professional goal is to be a conduit for each patient I encounter to have an opportunity where I can instill and they can learn that their understanding and behavior to participate in taking the prescribed medication does greatly impact not just their immediate health but also their quality of life. My passion for medication therapy management and compliances began in 1995 when I was the PIC, manager, and owner of Family Care Pharmacy located next to St.Dominic's Hospital (was sold to Kaiser) in Manteca, California. Prior to my ownership, I had managed St. Dominic's Outpatient Pharmacy at the same location until the hospital downsized and closed the pharmacy. I was approached and encouraged by physicians, discharge planners, and case managers to continue services by opening a pharmacy at the same site. I negotiated with the hospital and opened Family Care Pharmacy. I had developed and maintained professional relationships with the clinicians, nurses, emergency room staff, hospital and insurance nurse case managers, and social workers. Each presented cases of care and medication management challenges with their senior population due to such medication issues as non-compliance, side effects, misuse, medication expense, and caregiver problems and asked if I could help resolve these issues. I took their concerns to heart as it affected these patient's quality of life. Note that this occurred prior to the current MTM practice wave. I researched for possible solutions. I consulted with a colleague who recognized the above mentioned scenarios. He extended his experience and expertise. I then developed all aspects of a medication therapy management and compliance program including documentation and implementation of care plans that immediately responded and resolved the specific patient challenge. I received patient referrals from the above mentioned health care professionals who had expressed their concerns. Upon utilization of my MTM program for these senior patients, there was a decline in physician and ER visits as well as a decrease in costs to this senior group and the capitated risk pools demonstrated in and inspired by a MTM cost savings research project I initiated with a University of the Pacific School of Pharmacy professor. I enlisted and negotiated with a physician group capitated insurance manager a fee for the pharmacist MTM services and to access patient care costs prior to and after the MTM program implementation. The results indicated a average $800+ monthly savings to the risk pool after 1 month of MTM services. Details available upon an interview.
