I have worked as a pediatric nurse both in the acute care setting and the home care setting. In the acue care setting I functioned as a charge nurse in the pediatric intensive care unit at McAllen Medical Center. Areas I rotated within the hospital included NICU PEDIATRICS ADULT ICU and ER. I have had certification for PICC line insertion thru MMC and MCH Services. I also had NALS and PALS certifications. In the home care setting I worked with MCH services as a case manager which included caring for children with chronic and acute conditions and training of nursing staff to care for these children in the home setting. Duties also included generation POC to meet Medicaid guidelines initiation of care re-certifications discharges and coordination of patient care with all disciplines involved. I have also worked in home health with the adult and geriatric population since 1996. Duties in this field included general case management functions such as initiation of care resumption of care recertifications discharges and coordination of patient care with all disciplines involved. I am proficient in OASIS completion and coding for home health. I'am responsible for coordination the patient's health care needs with physicians and all disciplines involved in the patient's care. Other duties include supervision of LVN and CNA staff chart audits and formulating 60 day summary for physicians. The last two years I worked with Superior Health Plan as Care manager II. My duties involved visiting members (patients) adult and children for renewal and initial services they required such as PAS (provider) DAHS (adult daycare) and approval/denial of services as per assessment/observation of the member. Other duties included completion of the Medical necessity level of care assessment for use by TMHP. I have current RUG certification thru June of 2019.
